Tear Trough Filler — Ankara

Filling the hollow beneath the eye with hyaluronic acid. It works when a hollow is creating the tired look; it does not when the darkness comes from vessels beneath thin skin or from pigment. That distinction is made before treatment.

Three different causes of under-eye shadows

The same complaint can have different mechanisms behind it, and the right treatment changes accordingly.

The first is hollowing: a shadow created as fat support beneath the eye diminishes. This is the group for which tear trough filler works.

The second is vascular: vessels showing bluish through thin skin. Filler can partly reduce that shadow but is not a solution on its own.

The third is pigment: darkening of the skin colour in this area. Filler does not help here; skin care and sun protection come first.

Who it does not suit

  • Patients with marked under-eye bags
  • A tendency to eyelid oedema, with obvious morning puffiness
  • Active skin infection or inflammation around the eye
  • Pregnancy and breastfeeding

Where bagging is the main feature, filler can make the picture more obvious. Surgical options are assessed in that group and referral made where appropriate.

Treatment and risks

This is among the most delicate areas of the face for filler. Because the skin is very thin, filler is placed deep, close to the bone, and in small quantities. Filler placed too superficially can cause a bluish discolouration (the Tyndall effect).

Swelling and bruising are more common here than elsewhere and can last a few days. Because of the risk of intravascular injection, treatment should be performed by a doctor who knows the anatomy, with dissolving enzyme available.

Starting with a small amount and completing at a second session if needed is safer here than placing a large volume in one visit.

Recovery

  1. First 24 to 48 hours Swelling and any bruising are at their most obvious. Cold compresses help; do not massage the area.
  2. Week 1 Swelling resolves and the result begins to show.
  3. Weeks 2 to 4 The result settles. A second session is planned at review if needed.
  4. Months 9 to 12 The effect fades. Treatment is repeated if you wish to continue.

These are average times; recovery varies from person to person.

Will the shadows disappear completely?

It depends on the cause. Shadow from hollowing reduces markedly. Where the darkness is from pigment or vessels, the result is limited.

Will it cause puffiness?

Puffiness in the first days is expected. Persistent swelling can result from too much filler or placement in the wrong plane, and the filler can be dissolved in that case.

How many sessions are needed?

One session is enough for most patients. Because a conservative amount is preferred in this area, a second session is sometimes planned.

Will it help if my sleep is irregular?

Sleep and fluid balance affect the under-eye area considerably. Filler corrects structural hollowing; it does not remove fluctuation caused by lifestyle.
