What it can and cannot do

Hyaluronic acid filler is injected into the nose. Filler adds volume: it can fill a depression, flatten a profile by filling above and below a small hump, and give the tip a degree of support.

What it cannot do is make the nose smaller. Filler cannot narrow a nose, remove a hump, or refine a wide tip. Although it is often called "non-surgical rhinoplasty", it does not replace rhinoplasty; it is a different procedure doing a different job.

Who it may suit

• A small depression or irregularity on the dorsum
• Camouflaging limited asymmetry after rhinoplasty
• Cases where slight support of the tip is enough
• Patients who want to try a limited change before deciding on surgery

Filler is the wrong choice for patients who find their nose too large, want a hump removed, or have a marked deviation. In those cases filler makes the nose larger still.

Being clear about the risks

The nose is one of the highest-risk areas of the face for filler in vascular terms. If filler enters a vessel or compresses one, skin necrosis can occur, and vision loss from involvement of the ocular vessels has been reported, though rarely. This is a serious complication rather than a side effect.

What reduces the risk is a doctor who knows the anatomy, an appropriate technique, and dissolving enzyme kept available during the procedure. In a nose that has had previous surgery the vascular anatomy is altered and the risk is higher.

This is a medical procedure. It should not be performed by non-medical practitioners or outside a clinical setting.

How long it lasts

Hyaluronic acid is absorbed over time. The effect usually lasts nine to eighteen months, varying with the product, the area treated and the individual. If the result is not what you wanted, the filler can be dissolved with hyaluronidase. That reversibility is the single most valuable feature of the procedure.

Recovery

1. First 24 hours Mild redness and swelling are possible. Do not massage or apply pressure to the area.
2. Week 1 Swelling resolves and the result becomes clear. A review is recommended at this point.
3. Month 1 The filler settles. A small adjustment can be made if needed.
4. Months 9 to 18 The effect gradually fades. The treatment is repeated if you wish to continue.

These are average times; recovery varies from person to person.
